How care is managed when living in a rest home

If your need for care changes a reassessment is required to determine the new level of care you require. Your rest home should continually monitor your needs. If these change they should talk to you about this and make a referral for reassessment. You can also request a reassessment yourself at any time.  It is important to note that if you are reassessed as requiring a level of care which is not offered by your current care provider you will need to move to a facility which can provide the level of care you require.Updated 08-11-2010
